At Clarity HiFi, patient care is our top priority and protecting your privacy and safeguarding your personal information is just as important. This Privacy Statement describes the type of personal information that we collect, use, and disclose and how this information is used. It also explains the choices you have regarding such use and disclosure, and how you may access and update that information.

Personal information can be obtained in the following manners:

  • directly from the patient
  • from referring doctors or other health care providers
  • through our diagnostic testing and procedures

We will never collect, disclose, or use any of your information without your knowledge or consent. It is restricted to those individuals with a direct need-to-know basis only.

What information do we collect?

Personal and non-personal information that we collect from you is used for a number of purposes:

Personal information such as name, address, other contact information, financial/billing information, which may include credit card numbers and other such information, and insurance information (if applicable). To the extent we collect credit card information, it is done in compliance with Payment Card Industry Data Security Standards (PCI DSS).

Additional personal health information collected may include:

  • hearing history
  • medical history
  • records of previous visits
  • medications currently taking
  • results of diagnosis and testing
  • referral/specialists reports and recommendations
  • signed consent forms

How do we use your information?

We only collect, use, and disclose information about you in the following manner:

  • to enable us to communicate with our patients
  • to better understand our patients’ needs.
  • to assess your health needs
  • to advise you of hearing care options
  • to provide you with information about services offered at our clinics
  • to establish and maintain communication with you, including appointment scheduling and reminders
  • to communicate with other health care providers, including doctors/specialists involved in your care
  • to process credit card, cash, and EFT payments
  • to improve the content of our website or our clinic environment
  • to make business and financial decisions within our clinics
  • to comply with legal and regulatory requirements

Unless consent is given, we do not use or disclose your personal information for the purposes other than those for which it was collected.

How is your information stored and who has access to it?

Your information may be kept in hardcopy form (stored in secure areas) or in digital files, where it is encrypted on file servers in secure/access- controlled locations. Digital information is password protected and our systems are protected by industry standard IT security hardware and software measures. In the event of a breach, unauthorized access will be investigated.

We may enter into agreements with third-party providers specializing in data storage and protection. Sometimes that data is securely stored in the cloud, which may include locations outside of Canada. In those instances, only persons contractually obligated to secure and protect your data will be able to access that data. We will only enter into contractual agreements with providers which meet Canadian legal standards and requirements for storage and protection of personal health information.

Social Media

Clarity HiFi may utilize social media platforms such as Facebook, Twitter, LinkedIn Instagram, TikTok, and YouTube to market its services. Certain social networks allow ad targeting of users and the ability for users to share likes and interests with their friends/contacts. When you click on a social media plug-in, a link to our site may appear in the “Likes and Interests” section of your Facebook or other social media website profile. By using social media plug-ins, you consent to receiving communications from us for the purposes described above. Cookie/Tracking Technology and Interest-Based Advertising

This website may use cookie and tracking technology. Cookies are small data files transferred to your computer’s hard drive by a website. They keep a record of your preferences, making your subsequent visits to the website more efficient. Cookies may store a variety of information, including, the number of times that you access a website and the number of times that you view a particular page or other item on the website. Cookie and tracking technology are useful for understanding how visitors use our website. Please note that even if you opt-out of interest-based advertising by a third party, these tracking technologies may still collect data for other purposes including analytics and you will still see ads from us, but the ads will not be targeted based on behavioural information about you and may therefore be less relevant to you and your interests.

Electronic Communication

Subject to Implied Consent In some instances, your consent to receive email communications from us will be implied. For example, when you provide your email address to us prior to or at the time of your first contact with us, your consent to receive emails related to the procedure—e.g., appointment scheduling or receiving consent forms—is implied. When your consent is implied, we will only send you email communications that directly apply to the purpose of your original interaction with us, and your implied consent will automatically expire once the complete interaction with Clarity HiFi has been completed (i.e., after your procedure and/or after any/all follow-up appointments have been completed)

To successfully opt out, you must have cookies enabled in your web browser (see your browser’s instructions for information on cookies and how to enable them). Your opt-out only applies to the web browser you use so you must opt-out of each web browser on each computer you use. Once you opt out, if you delete your browser’s saved cookies, you will need to opt-out again. H2: Electronic Communication We may communicate via electronic means, such as email or SMS text message. By providing your email address or phone number to us, you consent “to opt in” to receive electronic communications from Clarity HiFi, subject to the guidelines and limitations described below. The option to “opt out” or “unsubscribe” after you have “opted in” will always be available.

Electronic Communication Subject to Express Consent

You must provide express consent if you wish to receive email communications regarding specific services, promotions, and surveys that you may be interested in. You may provide your express consent to receive such marketing messages by responding to a direct communication request for consent. Your express consent will remain in effect and will not expire unless you specifically request to “opt out” or “unsubscribe”.

Unsubscribing/Withdrawing Consent

We will always provide you with the opportunity to “opt out” or “unsubscribe” within each email or SMS communication we send to you.

Forwarding or Sharing Email Communication

If you choose to forward or share email communications that you’ve received from us, you assume the responsibility for the email communication and, therefore, Clarity HiFi will not be held responsible for any liabilities incurred as a result of that email communication being sent to another party.
